Accident Reporting and Investigation


This course covers how accidents occur, how to identify causes and how to investigate and write an accident report.

Course Content

  • Explain why workplace accidents need to be reported and investigated
  • Explain the concepts of multiple causation and root-cause analysis
  • Detail an incident investigation process using a hypothetical incident
  • Produce an incident investigation report based on a documented incident


  • Be 16 years of age or over
  • Applicants for whom English is a second language must meet IELTS (or equivalent) Level 5.0 (General Training). Proof of this will be required.

Minimum of 8 trainees and a maximum of 15 trainees per programme.


Course Fees

$380.92 + GST per person

Course costs include all learning materials and completion certificate. Costs also include registration of unit standard credits with NZQA.
